Overview
PHC brings medical care directly to individuals through street outreach and street medicine programs in Santa Clara County and across the Peninsula. For people experiencing homelessness, getting to a clinic isn’t always possible, so we go to them. Our teams meet individuals where they are, whether that’s an encampment, a shelter, or a quiet corner of the community, and deliver care without the barriers of transportation, lengthy paperwork, or long waits.
On the ground, our teams provide basic medical care, preventive services, and essential supplies while building trust with people who have often been overlooked. That trust is what opens the door to lasting change. Over time, these relationships become a bridge to something more stable, connecting individuals to ongoing healthcare, housing resources, and the broader network of support they need to move forward.
