Our mission is to provide accessible, affordable healthcare for people who are underserved, at-risk, or experiencing homelessness — and to meet them where they are.

Traditional care isn’t built for the realities of poverty and homelessness. People navigating unstable housing need integrated care that treats the whole person — physical, mental, and emotional health together. PHC’s model meets that need: client-centered care paired with help enrolling in government aid and community support, so people can thrive.

Built over 20 years.
Peninsula Healthcare Connection began in 2006 as a collaboration between the Community Working Group, Palo Alto Medical Foundation, and the Stanford University School of Medicine. From a single clinic at the Opportunity Center in Palo Alto, we set out to make quality care reachable for people the traditional system overlooks.
Twenty years later, that mission still drives us. Through lasting partnerships with Stanford Health Care and Palo Alto Medical Foundation, volunteer physicians bring their expertise to our patients in the clinic and on the street.
Beyond the clinic.
Our model delivers care in the clinic and directly in the community, reaching people who face social, financial, and physical barriers to getting help. By closing those gaps, we improve health outcomes across the region.
